2019-20 School Year: Medical History and Plans
Your child's Medical History and treatment protocol is very important to the School Nurse and any other medical professional that may need access in case of an emergency.

General - Required For All Students:

  • All parents are required to complete a Medical History Form at the beginning of every school year.  Form can be accessed on your MyNewmark Parent Portal.
  • If your child requires any medication during the school day or any after school-related activity, a Medication Release Form* must be completed.

Sports Related - Required For All Students Participating in Newmark Athletics

Newmark K-8 Interscholastic Sports
Athletic program includes basketball (Fall) and kickball (Spring).
According to NJ State Guidelines, in order to participate in any interscholastic sport, students must receive a physical examination each year.  Preparticipation Forms include:

Newmark High School Interscholastic Sports
Athletic program includes cross country and soccer (Fall) and basketball and track & field (Spring).
According to NJ State Guidelines, in order to participate in any interscholastic sport, students must receive a physical examination each year.

Mid-Season Forms
For all students who participated in a Fall sport during the 2019-20 School Year and also plan to participate in a Spring sport, please complete a Health History Update Questionnaire as mandated by the State of New Jersey Department of Education:


*Must be signed by Physician
